There are several verizon "global" phones that have GSM and CDMA on the same phone. I would think the data plan applies to verizon phones in GSM locations.
Nope, it's explicit about only being for CDMA (presumably IS-95 and its successors) roaming. This is in the first sentence on the screenshot.
